Position Summary:The PACE RN Supervisor has the responsibility for providing direct and indirect nursing care to PACE patients as well as administration and supervision of the PACE nursing team including Clinic RNs, Day Center LVNs, MAs and CNAs. Works under the supervision of the PACE Medical Director and day to day supervision of the Associate Medical director but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which he/she is responsible.Essential Functions of the Job: Monitor the day to day RN activities in the PACE Clinic and Day Center. Develop s and implement s appropriate/required PACE nursing protocols and monitors that nursing team is following those protocols. In coordination with the PAC E Medical Director and the PACE QA Manage r, ensures that clinical protocols are followed, reviewed and updated as needed and regular audits are conducted; demonstrates active participation in PACE QA processes; is involved in assessment and implementation of QA improvement plans. Conducts regular audits to ensure the PACE clinical area is meeting PACE requirements and regulations Assists with the planning and preparation of the PACE annual CMS audit. Participates as a member of the Interdisciplinary Team; attends staff meetings and takes part in participant care planning. Leads and coordinates Nursing Team Meetings, trainings of nursing team, and on-going review of policies and procedures. Responsible for assessing and implementing nursing team competencies to all new hired nursing team members. Conducts and reviews PACE participant assessments as needed. Initial and periodic assessments (minimally every 6 months) to be completed prior to the scheduled team meeting Ensures the development of a plan of care based on nursing process and incorporates the plans of other disciplines and continuing care needs; includes the patient/family in developing or revising the plan of care; makes referrals to multidisciplinary support services. Communicates participant changes to team members; coordinates participant care with day center nursing team. Conducts homes, hospitals, Skilled Nursing Facility, etc. visits and assessments as requested/needed. Coordinates with Day Center lead for any medical center needs of participants attending the day center. Ensures that all clinical logs are completed in a timely manner Performs advanced nursing assessment using critical thinking skills. Ensures the care provided conforms to accepted practice standards; provides correct treatments/procedure s and other care as prescribed and according to patient care standard s; Ensures participant and family education is provided by the nursing team as needed Is responsible for coordination of 24 hour care delivery including coordination of after hour/weekend/holiday on call schedule ; Identifies emergency situations and independently initiates advanced therapy Acts as coordinator of patient care with other health care personnel and evaluate s patient care measures instituted; coordination of care and discharge planning with hospitals and skilled nursing facilities; understands and demonstrates respect for patient rights and utilizes established mechanisms for management of ethical issues in patient care. Records patient care delivered as planned and any variation, with appropriate rationale; makes and records observations related to impending or associated problems; implements nursing measures related to impending or associated problems. Acts as the PACE Clinic Lead RN and delegates nursing care to appropriate personnel; is responsible for overall performance and timesheet management for PACE Nursing Team, Integrates cost effective measures into nursing practice; Recognizes unit problems and takes responsibility for documenting same ; Complies with hospital expectations to meet staffing demands based on patient care needs; Complies with policies addressing safe working conditions; monitors unsafe working conditions; recognizes inappropriate and /or ineffective patient care management, resolves issue/ problem and completes written reports; fulfills call for unit as assigned and adjusts staffing for census (volume) and acuity.Job RequirementsEducation Required (Minimum level of education):Graduate of an accredited school of nursing.Bilingual:(Spanish/English)Licensure, Registry or Certification Required:Currently licensed in CA as a Registered Nurse license; must present credentials which demonstrate ability to meet performance criteria for RN.Experience Required (Minimum level of experience):Has practiced nursing in the last year; 1 year RN experience with at least 9 months experience working with the frail elderly population at PACE.Preferred Experience: Knowledge and or experience working in senior or elder care setting - PACE, Nursing Home, Hospice, ADHC. Nursing competency and familiarity with care paths, interact tools commonly used in skilled nursing home settings. Must be able to triage clinical symptoms, apply care paths and clinical interactive tools in patient evaluation and care prior to communications with physicians. Must be comfortable starting IV lines, IV medication administration and monitoring. Must be comfortable with acute care of frail adults, complex wound care and cognitively impaired adultsVerbal and Written Skills Required to Perform the Job:Excellent oral, written, and interpersonal communication skills.Technical Knowledge and Skills Required to Perform the Job:Must possess the skills to provide care for geriatric patients, must thrive in team environment, must possess good organizational and supervisory skills, ability to effectively handle difficult and unusual interpersonal situations. Meet a standardized set of competencies for the specific position description established by San Diego PACE and approved by CMS.Equipment Used:Minor medical equipment such as vital signs monitors, emergency cart, automated external defibri llator (AED), and patient personal assist devices.
