Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members with
physical needs.
Key Responsibilities
Evaluate member needs and recommend care plans to achieve optimal
healthcare outcomes
Coordinate care between members, families, and providers to ensure
access to appropriate services
Monitor member progress and update care plans as necessary to meet
individual goals
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
Registered Nurse (RN) state licensure or Compact State Licensure
required
