Salary: $155,000 - $170,000 per year A bit about us: We’re a

training and support Job Details Conduct Medicare annual health
assessments in clinic and at patient homes See 7–12 patients a day
Perform H&Ps and document in EMR systems Collaborate with
patients’ PCPs to ensure continuity of care Requirements Active NP
license in California Bilingual (Spanish preferred) Comfortable
with EMR systems and electronic documentation Professional,
flexible, and passionate about patient care Strong team player with
