Pay Rate: $18.27-$28.85/hr

Hours: M-F 8am-5pm

candidates will train at the corporate office for 2 months (SY
Corp. Office - 1600 Precision Park Lane, San Ysidro, CA 92173)
PAYRATES Min$18.27 Mid$22.84 Max $28.85
Mileage is included for this position as well as a generous comp
plan.
Candidates must be willing to commute to appointments all over San
Diego County and Vista area to meet with patients to enroll in
program.
Enrollment Specialists will have quotas to meet per enrollment.
Position is sales related and administrative for each enrollment
completed.
Must be KNOWLEDGE OF MEDICARE/MEDICAL

San Ysidro Health. Position Summary:
The PACE Enrollment Specialist is responsible for the initial
screening and coordination of new participants to the San Diego
PACE (Program of All Inclusive Care for the Elderly) program.
He/She is also responsible for working with the PACE Marketing
Manager to implement the programs intake, enrollment, outreach and
marketing activities.
Essential Functions of the Job:
Conducts San Diego PACE program eligibility screening, intake,
education and enrollment assistance activities to initially
determine participant needs and eligibility for enrollment.
Coordinates with potential participants and/or their families to
conduct initial screenings and assessments in the homes of the
potential participants.
Completes appropriate documentation related to participant
assessments as required.
Coordinates communications of eligibility decisions, approvals or
disapprovals with participant or their family members.
Participates in the interdisciplinary team (IDT), management team
and other related meetings and activities as needed.
Participates in outreach and marketing activities in conjunction
with the PACE Marketing Manager.
Collects and data enters the daily referral, intake, enrollment,
marketing and outreach activities notes with submission of weekly
and monthly reports to PACE Marketing Manager.
Provide tours of PACE center(s) to interested family members,
potential participants, community partners and providers.
Facilitates referral from SYHC and community resources for
assistance with other needs and complex issues.
Provides responsive, high quality service to patients, SYHC
employees, representative of County and outside agencies and
members of the public by providing accurate, complete and
up-to-date information in a courteous, efficient and timely
manner
Provides input and feedback on PACEs internal and external
outreach, education and enrollment plan for engaging, assisting,
enrolling and retaining new and current patients.
Conducts public outreach and education activities, individually and
in group, to ADHC, SYHC patients and community members at clinic
locations and other community locations about PACE and other
related health coverage options. Additional Duties and

Work independently and as effective member of the team.
Multi-task in projects and their respective activities, timelines
and issues.
Able to adjust outreach and enrollment strategies as necessary
based on data analysis.
Establish and maintain effective interpersonal relationships with
all levels of staff, other departments, agencies, providers and the
general public.
Complies with all department, organization and government policies
& procedures.
Attends meetings and trainings as required.
Adheres to and models SYHCs core values and behaviors of

Adheres to SYHC and PACE attendance and punctuality policies and
practices.
Performs other related duties as assigned or requested. Job
Requirements:
Education required:
High school diploma or GED equivalent. .
Education Preferred:
Bachelors Degree in a health or human service field. Experience
Preferred:
A minimum of one year working in community health, senior health,
marketing, or enrollment for a public health, social service
organization or health plan.
One year of MediCal, Medicare, managed health care eligibility or
senior related sales.
Working with the frail or elderly population is a plus.
Certifications/Licenses:
CA drivers license with appropriate automobile insurance
required.
Must take and pass the State of California DHCS PACE and Medi-Cal
Marketing Certification exam within 60 days of hire. Verbal and
Written Skills to Perform the Job:
Must be able to communicate effectively and comfortably in public
forums.
Prepare concise and clear reports on marketing and outreach.
Bilingual (English/Spanish) is preferred. Technical Knowledge and
Skills to Perform the Job:
Good knowledge of Microsoft Word applications including Excel,
Access and Power Point.
Computer data entry, analysis and reporting experience is
required.
Knowledge of electronic health data systems including NextGen is
preferred. Competencies:
